Forms are to be filled out and submitted via Chancellor's University Report. When this is not feasible, please submit a brief action item in the CUR, with the clause "a copy of full proposal is available at the OAA." Then mail completed forms, in duplicate, with a reference to the CUR month and item number (!), to the attention of the Director of PRogram of Review.
