Affiliation Agreements

Clinical sites with a Pre-Negotiated Agreement now in place OR with negotiations pending are listed below.

If you wish to establish an agreement for a program with a clinical site on either of these two lists, send Yvette Santana-Prado at the Office of the General Counsel an OGC Approval Form and she will then generate the appropriate agreement and send copies to you to forward to the site for signature, OR she will advise you that there will be a delay because the pending Pre-Negotiated Agreement with the site has not yet been concluded.

As pending agreements are concluded and negotiations with other sites are begun, the two lists will be updated accordingly.
